PAM Health provides a comprehensive benefits program that offers a range of resources and is an important part of your total compensation. Some benefits are paid in full by PAM Health, and some costs are your responsibility. You can view the Employee Benefits Guide online here.

When Can I Enroll?

Definitions

Full-time Employee

Regularly scheduled to work 30 or more hours per week

Part-time Employee

Regularly scheduled to work 24-29 hours per week

Dependents

Spouse and eligible children (varies by benefit)

Benefits Available To All Eligible Employees

Vacation – 15 days per year earned for FT, 7.5 days per year earned for PT with less than 5 years of service.

Holidays – Six 8-hour paid holidays for qualified employees

Sick Leave – 10 days per year earned for FT, 5 days per year for PT

N

Bereavement – Up to 24 hours paid absence for death within immediate family

N

Jury Duty – The difference between the jury duty allowance and normal daily wage (up to 20 days)

Education & In-Service Opportunities – Education Advancement Program to include CEU’s and tuition assistance

N

Voting Absence – Two hours off with pay if work schedule prevents voting in regular election

401K Plan – Contribute up to 100% of your earnings within IRS limits. Company match is discretionary and determined at the close of each plan year.

Verizon & AT&T Discount – PAM Health Employees receive up to a 24% discount on monthly cell phone bill

Medical Benefits

PAM Health offers a choice between an EPO and 2 HDHP/HSA plans.  The cost is split between you and PAM Health.  Prescription coverage is included with the medical plan. The medical plans also include access to TeleDoc for 24/7/365 access to medical care through telephone or video consultations with licensed practitioners.

Dental Benefits

Comprehensive indemnity dental insurance. Premiums are paid by pre-tax payroll deduction.

Vision Benefits

Dual benefit vision plan combines discount program with scheduled annual benefits. Premums paid by pre-tax payroll deduction.

Rx `n Go

No cost prescriptions for nearly 800 generic medications, 90-day supply delivered directly to your home

Employee Assistance Program (EAP)

Unlimited access to a toll-free number 24/7 and up to 3 counseling sessions per year, per incident

Health Savings Account (HSA)

Pay for out-of-pocket health expenses for you and your dependents with the account funded by pre-tax payroll deductions.

Who is eligible: Full-Time/Part-Time Employees & Dependents enrolled in HDHP/HSA plans only

Life Benefits

Group Term Life: Term Life insurance in the amount of 1 ½ x annual salary. Max benefit of $150,000.

Supplemental Term Life & Voluntary Whole Life: Purchase life insurance for your self and dependents at low group rates. Premiums paid by after-tax payroll deduction

Flexible Spending Accounts (FSA)

Medical: Pay for out-of-pocket health expenses for you and your dependents with the account funded by pre- tax payroll deductions. Use it or Lose it rule.

Limited Purpose: Pay for out-of-pocket dental & vision expenses only.

Dependent: Pay for childcare expenses for your children up to age 13 with this account funded by pre-tax payroll deductions.

Disability Benefits

Short-term (STD): Income protection at low group rates. Premiums paid by after-tax payroll deduction

Long-term (LTD): 40% of your salary is paid following 180 days of disability with buy up option.
